You are hereby notified that an action has been commenced against you in the Circuit Court of Johnson County, State of Missouri, the object and general nature of which is to determine that you are unwilling, unable, or unfit to provide support and care of the above-named juvenile and appoint a successor guardian over the above-named individual.
The names of all the parties to said action are stated above in the caption thereof and the name and address of the petitioner's attorney is Dominic S. Sherwood, Attorney-at-Law, 125F North Holden Street, Warrensburg, MO, 64093.
